Health risk assessments (HRAs)

Rendered on a tablet in the waiting room OR captured from patients over the phone. These valuable assessments not only generate needed revenue for the practice, but also are tremendously valuable for patients.
CareONE’s proprietary “Clinical Trackstm” are generated after each health risk assessment and are a personalized sequence of services, procedures, treatments and referrals that are catered to each individual patient based upon their health history. The Clinical Tracks are all Medicare-approved covered services and are recommended by Medicare for patients whose health issues show medical necessity for such services

Comprehensive medication reviews (CMRs)

Our State-licensed pharmacists are available to provide in-depth reviews of patients’ medications to help them arrive at meaningful decisions about effectiveness, cost and adherence. Patients today have many questions about their medications and often their primary care physicians do not have the time to sort through a long and complicated list of meds to get to the patients’ questions.
Our pharmacists assist the practice’s physicians by performing the screening and discovery with the patients about all things involving their medications.

Pain Management

Pain Management can be daunting for primary care physicians who have time constraints keeping up with things like Urine Drug Screens (UDS), Opioid Risk Tool (ORT) and Current Opioid Misuse Measure (COMM).
